TO BOOK AN APPOINTMENT
  1. Before booking an appointment, you’ll need a referral from your GP or another specialist to see Dr Boothroyd. A referral from a general practitioner will be current for 12 months from the date of the first appointment unless otherwise stated. A referral from another specialist will be current for 3 months from the date of the first appointment.
  2. Once you have a referral, email your referral to bookings@ivfmed.com.au
  3. We’ll contact you by email or SMS offering an appointment time.
  4. Once an appointment is arranged with Dr Boothroyd, you will be emailed a form to complete before your appointment.
  5. It is important that you do not see Dr Boothroyd whilst you are undertaking a treatment cycle at another clinic because Medicare may not reimburse your medical costs.
  6. If you are late or going to be late for your appointment we ask that you phone ahead to reschedule to avoid inconvenience for other patients.

To minimise the risk of Covid-19 infection for staff, only attend your appointment in person if you and your partner are well, have completed the covid questionnaire and wear a mask. Appointments can be virtual, unless you are being examined.
