DR CLARE BOOTHROYD

1. Before booking an appointment, you’ll need a referral from your GP or another specialist to see Dr Boothroyd. A referral from a general practitioner will be current for 12 months from the date of the first appointment unless otherwise stated. A referral from another specialist will be current for 3 months from the date of the first appointment.
2. Once you have a referral, email your referral to bookings@ivfmed.com.au
3. We’ll contact you by email or SMS offering an appointment time.
4. Once an appointment is arranged with Dr Boothroyd, you will be emailed a form to complete before your appointment.
5. It is important that you do not see Dr Boothroyd whilst you are undertaking a treatment cycle at another clinic because Medicare may not reimburse your medical costs.
6. If you are late or going to be late for your appointment we ask that you phone ahead to reschedule to avoid inconvenience for other patients.

What happens in the first consultation?
Your initial consultation will be approximately an hour or an hour and a half if your partner attends.

Telehealth (consultations over the phone) are available and will be split between two appointments – one of shorter duration by telehealth and one of equal duration when you attend face to face with Dr Boothroyd.

In order to be able to provide an individualised care, you will be asked to bring the following:

• a summary of the questions you would like answered
• your past medical history
• past ultrasound images or x rays including any operation notes or medical summaries you may have
• family history
• a list of the medications you are currently taking.

If you are transferring your care from another specialist it is preferable that the records from your previous specialist are available at the time of your consultation. You can obtain copies of these yourself from your specialist or sign a release note and Dr Boothroyd’s reception staff will obtain them in advance of your first visit.

When booking your appointment, you will be notified of out of pocket costs and Dr Boothroyd's reception staff are happy to discuss these with you.

Sometimes additional costs may be incurred e.g. for an ultrasound (a scan), pathology testing or a colposcopy (a procedure performed for the investigation of an abnormal cervical screening test).

If you are having a surgery you will be asked to pay your invoice prior to your treatment and an itemised account which can be submitted to Medicare (and if applicable, your private health fund.) Billing for IVF treatment is through Care Fertility Greenslopes.

If you are experiencing financial hardship please discuss this with Dr Boothroyd or the reception staff.

If you need to cancel or change your first appointment, we ask that you give at least five working days notice. This allows us to offer the appointment to another new patient on the waitlist. If you are not able to give sufficient notice of cancellation of a first appointment another appointment time cannot be guaranteed.
